ADMISSION

ADMISSION REQUIREMENTS

PRE-SCHOOL & GRADE SCHOOL

  • Certified true copy of Report Card (Not Applicable for Nursery)
  • Recommendation Letter (Not Applicable for Nursery)
  • Four copies of 2x2 I.D. picture
  • Baptismal and Birth Certificates (Photocopy and Original)
  • Age Req't by June: Nursery - 3 1/2 to 4; Kinder - 4 1/2 to 5; Prep - 5 1/2 to 6;
    Grade 1 - 6 1/2 or 7
  • Report Card (Form 138) without any failing mark from first grading to the last grading period (for transferees only).
  • One long brown envelope

HIGH SCHOOL

  • Entrance Examination
  • Certified true copy of Form 138 (Grade School Report Card)
  • Certificate of Good Moral Character
  • Five copies of 1x1 size picture
  • Baptismal and Birth Certificates (Photocopy and Original)
  • One long brown envelope

COLLEGE

NEW STUDENTS:

  • Form 138-A or High School Report Card
  • Certificate of Good Moral Character
  • Police, NBI CFI Clearance may also be asked on special cases.
  • NSO Birth Certificate (Photocopy and Original)
  • Two (2) copies of 2x2 I.D. pictures
  • Photocopy and Original copy of Mariiage Contract (for married students)
  • Result of placement test in the university

TRANSFER STUDENTS:

  • Honorable Dismissal
  • Certificate of Good Moral Character
  • Interview for recommendation with concerned Dean
  • Test result from the Guidance Center
  • Police, NBI clearance
  • NSO Birth Certificate (Photocopy and Original)
  • Photocopy and Original copy of Mariiage Contract (for married students)

FOREIGN STUDENTS

  • Letter from the Director of Foreign Affairs.
  • Original copy of Birth Certificate
  • Personal History Statement (not more than 200 words).
  • Resume/Profile.
  • Application for Non-Immigrant Visa.
  • Transcript of Records/Authentication of Grades from the previous school.
  • Notarial Certificate of Non-Criminality.
  • Certificate of Scholarship from the Sponsoring Agency (if any).
  • Bank Certification.
  • Passport with Student Visa (Photocopy - clear passport ID).

GRADUATE SCHOOL

  • Original Transcript of Record
  • NSO Birth Certificate
  • For married students, submit a copy of their marriage contract.
  • Result of placement test in the university
